Capital Area Healthy Aging Coalition

The Capital Area Healthy Aging Coalition brings partners together to strengthen healthy aging efforts across the region, including chronic disease prevention, falls prevention, caregiver support, and opportunities that reduce social isolation among older adults.

Who We Are

The Capital Area Healthy Aging Coalition is a collaborative group of healthcare providers, social service agencies, municipal leaders, senior-serving organizations, housing partners, emergency responders, and community members.

The coalition operates through shared leadership among community partners, with coordination support from the Capital Area Public Health Network (CAPHN).

Guided by the Healthy Aging priority area of the Community Health Improvement Plan (CHIP), the coalition focuses on collaboration, coordination, and shared problem-solving to strengthen healthy aging across the region.

What We Do

The coalition works collaboratively to advance healthy aging priorities identified in the Community Health Improvement Plan. This includes efforts to:
Improve participation in local programs and services for chronic disease prevention and management
Prevent injury, disability, and death caused by older adult falls
Increase capacity to provide services and supports for family caregivers
Reduce social isolation by promoting opportunities for social engagement
A smaller planning team supports strategy development and coordination of coalition activities.

WHY THIS WORK MATTERS

As the population ages, communities must adapt to meet changing needs. Coordinated efforts across healthcare, social services, and community organizations help older adults remain safe, connected, and supported, while strengthening the systems that families and caregivers rely on.

MEETING INFORMATION

The Capital Area Healthy Aging Coalition meets monthly. Meetings typically include partner updates, discussion of regional data, and collaborative planning.
